Accepting an orphan request remove co-maintainer instead of promoting them

Description

Accepting an orphan request also remove co-maintainer instead of promoting them to maintainer.

Reproduction

Accept an orphan request of a package that have at least one co-maintainer, see both the maintainer and the co-maintainer being removed. Expected behavior would be that the first co-maintainer be promoted to maintainer.

The bug is present on v6.2.4 but I know it was present on earlier versions as well. @artafinde said it's a regression bug as it used to work before, however I don't know which specific version introduced this regression unfortunately...
